Please ensure the online intake/consent and direct billing forms are filled out prior to your appointment.
We provide direct billing for most insurance providers (not all).
We require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee to the card on file. The fees for late cancellations or missed appointments is 100% of the treatment amount.
